All testing materials are for in vitro non-diagnostic research use only. Use of any testing materials to determine therapeutic options for patient treatment or any other diagnostic purpose is prohibited. The Institution will be entitled to one order of testing materials. The Institution is prohibited from charging any patient, entity or third-party payer for any testing materials or including the value of the testing materials on any cost report submitted to any government agency or other third party. The Institution may not transfer any materials provided through this program, including AZTREONAM / AVIBACTAM gradient strips, to another party or laboratory. Institution represents and warrants that it holds all required licenses, permits, registrations, accreditations, approvals, exemptions, and authorizations to receive, handle, use, and process the testing materials. The Institution acknowledges that the provision of testing materials through this program are not contingent on, nor intended to serve as an inducement or reward for, any past or future purchases of Abbvie products. Abbvie reserves the right to disclose any transfer for value provided in connection with this program in accordance with state and federal transparency laws, rules, regulations, requirements, and industry codes. Abbvie makes no representations, warranties, or guarantees as to the testing materials, including with respect to their adequacy, performance, quality, fitness for a particular purpose, or usability, and the testing materials are provided “as is”. Institution is solely responsible for obtaining all equipment, personnel, supplies, consents, and any other necessities to obtain, run, and process data from the testing materials. This program may be discontinued at any time.
